The OnTrack team is a group of optimists that believe we can be part of creating a “Golden Age of Healthcare Value”. We believe there are innovative leaders in the healthcare industry that want to foster better outcomes at a lower cost; not simply to adapt to changing reimbursement models, but because it is the right thing to do. We believe providers that pursue better patient outcomes through the efficient use of resources will build the future of healthcare.
Now is a unique time in history to make giant leaps rather than incremental changes to healthcare delivery. Multiple forces are converging at the same time, providing opportunities for heart failure, COPD, diabetes, obesity, mental health, and other chronic disease management programs that did not exist even two years ago.
Proven health regimens improve patient outcomes. Implementing these treatments consistently and effectively has been logistically difficult. OnTrack is committed to make managing patient health regimens more consistent and effective. The rapid adoption of ubiquitous mobile devices by patient populations managing one or multiple chronic diseases means we can now engage the patient whenever and wherever they are.

Paul Luetmer, MD, FACC, Chief Medical Officer
As a clinical cardiologist and Medical Director of the Aspirus Heart and Vascular Institute in Wausau WI, Paul was charged with improving outcomes and reducing costs for patients with congestive heart failure. Paul developed and executed a pilot project for heart failure management, integrating the resources of the hospital, clinics, visiting nurses, pharmacy, dieticians, nursing homes, and hospice. While this was quite successful, shortening length of stay, reducing readmissions, and improving patient satisfaction; it was immediately apparent that patient engagement, compliance with the medical regimen, patient education, communication, and monitoring of symptoms and physical parameters would be critical to further improving outcomes. Realizing the potential of a mobile health application to resolve these issues, Paul and his partners founded OnTrack to Health for this purpose.
