Initial adoption of CMS’s CCM services by providers was slow for good reasons: The initial CCM billing code (99490) rolled out by CMS in 2015 was cumbersome to bill, had low reimbursement, and included technical requirements that kept 98% of providers from participating between 2015 and 2017.
